
-Situational Assessment-
A plan is written which consists of standard and specific questions regarding performance that the funding source counselor wants assessed. The situational assessments questions and plan are written prior to the assessment beginning. The job coach is given the S/A questions and makes progress notes on the areas being assessed, then makes a weekly summarization on the situational assessment report form. At the end of the assessment, the Employment Specialist will write the information gathered into a summary report for the referral counselor.
