Program Mission:

Our Community Support Program is designed to educate consumers and bring people together so they will learn how to sustain with in the community of their choice. MVRA case managers are a resource to assist individual's access services and learn to self advocate. This program incorporates community supports as well as offers a vocational program which is often an integral component that is missing in other programs.

Program Description:

MVRA's Clinical Case Management team engages adults, who have mental health and co-occurring disorders, to access community resources. Case managers assist clients in developing personalized and manageable goals. The program centers on the following aspects of life with focus on client's choice: i.e. Housing, Finances, Basic needs, Spirituality, Social Relationships, Emotional and Physical Wellness, Sexuality, Intellectual Pursuits, Vocational skills and goals leading to direct integrated job placement, Staff will be responsible for assessment when applicable, case documentation, referral when necessary and thorough communication with all other provider agencies.

Case Management Services:

Eligible criteria for Case Management services include the following:

Case Management Services are provided by Licensed Staff.

Fees:
Mainecare, Cash & qualifying private/commercial insurances are accepted upon approval of insurance benefits & verification.

A sliding scale fee is addressed on a case by case basis.

Monday – Friday 9am – 5pm
