Clinical Case Management
(Also known as Community Integration Services as defined by the State of Maine.)
We are here to help you coordinate your needs in life. We help consumers connect to services and actively engage with service providers in such areas as medication management, therapy, housing, Vocational Rehabilitation, and much more. We are able to assist with assessments, planning, evaluations, and help monitor your well-being. Our case managers will be here to help you navigate multiple complicated systems while advocating for you and your specific needs and helping you to increase independence with confidence. Services are individualized and in accordance with an Individualized Treatment Plan and are provided by certified and experienced staff.
